There may be nothing wrong with the design of the signal system.
Some people say there is a quality issue.

Design and reliability are two different things.
A signal system with a very good design, can still fail from time to time. The important thing is to make sure the procedure of dealing when a failure occurs, e.g. manual control at at slower speeds.

The subway was under manual control at that time. But the driver of the second train made a mistake of not stopping in time.

It was never about the signal system.

With manual control in place, the driver is supposed to drive the highspeed train at very slow speed and visually make sure there is no train ahead, instead of relying on the colour of the signal lights.
