It looks like there is no reinforcement in the foundation of the railway.
Drainage system is also poor under the railway.

Yup it is... But, in this case it's understandable...

1) Ageing
2) Too close to the river bank
3) Not main route
4) Old standard... etc.

Like this time, I believe this video is taken by a rehabilation crew...

BTW, I have to remind you that monitoring is so important for construction... do you know why there are so many casualities on these types of events in China?

Building railway in those China remote areas are tough...

Indian railways are not maintained well at all, therefore the frequent accidents.
