if a person is not a MTR worker
and this situation, Will he/she talk to the mother/ or talk to MTR workers
ppl don't want to get involve of any cases/situations....because they have to go to court ...

Yes, that's the point. People (most people) are SELFISH!

Eating inside MTR train cabin is violating the MTR regulations (rather than saying breaking the legal law), same as peeing in any spot in the MTR station (since the whole MTR station is not a public washroom).

It's like, when you see dog owners not picking/cleaning up after dogs, would you talk to/stop them? (and actually they're breaking the law instead of private regulation) Oh yes, it's gonna waste your time, dog owners might yell back at you with swear words or might punch you right on your face, their dogs might get aggressive and try to bite you..... , so what's the consequence? Dog ****s all around the city! Then, isn't it (only) the government/city responsibility to keep the whole city clean?
Yes, even you ask that dog owner to pick/clean up after his/her dog, he/she might not doing the right thing in many other locations if he/she doesn't want to. So, you rather not telling him/her to do the right thing?

Well, what is the "right thing to do" might be varies to different people.

It's parents' responsibility.
If I were the parent, I will take my kid to washroom before transiting. (of course, having body sickness is out of the issue.)

Yes, people should follow the laws as well as regulations established by any company when they agree to use their service. However, when you see people not following, what would/should you do?

at that situation, MTR workers/stuffs should stay in the train for safety.......
or use camera to record what had happened in the train

to miniize the crime at trains

Same case may happen to other public transportations.
So, a staff or surveillance camera might need to apply to all mini-vans, buses (both upper and lower decks), MTR trains (every cabin), KCR trains (every cabin), trams (both upper and lower decks), ferries (all decks), and airplanes....... etc.
Moreover, non-disciplined people may also pee and poo in public gardens, flower beds, swimming pools, lakes, reservoirs, on any road and in any street..... etc, how practical can it be done? And how much money is gonna spend on this idea, and then how much more tax would citizens/residents need to pay?
